While glaucomatous optic neuropathy may be the most common acquired optic neuropathy seen in clinical practice, it’s certainly not the only one. Moreover, some non-glaucomatous optic neuropathies are not only sight-threatening, but potentially life-threatening. Making the correct differential diagnosis is critical but challenging, perhaps particularly with optic nerve cupping in the absence of elevated IOP. This lecture will review the most common conditions that mimic glaucoma, highlighting features revealed by case history, clinical examination, and ancillary imaging (that is, the entire clinical picture, not simply ONH morphology) that facilitate the recognition of non-glaucomatous optic neuropathies, and help avoid the potentially serious consequences of misdiagnosis.
